Supported Living

Supported Living at Bloom Community Supports LLC provides individualized assistance to help people with intellectual and developmental disabilities live safely, independently, and meaningfully in their homes and communities.

Services are based on each person’s individualized needs, preferences, strengths, goals, and service plan. Our trained staff provide person-centered support designed to promote independence, dignity, choice, confidence, and community inclusion.

Supported Living may include assistance with:
• Daily living and self-care activities
• Meal planning and meal preparation
• Household organization and routine tasks
• Personal care and hygiene support
• Transportation and community access
• Community participation and social activities
• Safety awareness and emergency preparedness
• Communication and interpersonal skills
• Decision-making and problem-solving
• Medication reminders or related support, when permitted and applicable
• Building and maintaining independent living skills
• Developing routines that support greater stability and independence
• Supporting goals identified in the individual’s service plan
• Encouraging meaningful relationships, activities, and community involvement

Our staff work alongside each person rather than simply completing tasks for them. The goal is to help individuals strengthen their abilities, make informed choices, participate actively in their daily lives, and maintain as much control and independence as possible.

Adult Habilitation

Adult Habilitation at Bloom Community Supports LLC is designed to help individuals with intellectual and developmental disabilities build, maintain, and strengthen the skills they need to live as independently and meaningfully as possible.

Adult Habilitation may include support with:
• Daily living and self-care skills
• Communication and social development
• Decision-making and problem-solving
• Personal responsibility and independence
• Community participation and inclusion
• Building healthy routines
• Developing interpersonal and relationship skills
• Learning appropriate safety awareness
• Increasing confidence in community settings
• Practicing household and independent living skills
• Supporting personal goals identified in the individual’s service plan
• Encouraging meaningful activities, interests, and community involvement

Our staff work alongside each individual at a pace that reflects their abilities and goals. The focus is not simply on completing tasks for the person, but on helping the individual develop skills, make choices, participate actively, and become more confident in daily life.

Placement Services

Placement Services at Bloom Community Supports LLC are designed to help individuals with intellectual and developmental disabilities access a safe, appropriate, and supportive living arrangement that reflects their individual needs, preferences, goals, and level of independence.

Our team works to support a thoughtful placement process by considering the person’s strengths, support needs, lifestyle preferences, safety needs, compatibility, and desired living environment. The goal is to help each individual live in a setting that promotes stability, dignity, choice, community participation, and quality of life.
 

Placement Services may include support with:

  • Assessing the individual’s residential needs and preferences

  • Identifying appropriate living arrangements

  • Supporting placement planning and transition into a new home

  • Reviewing safety, accessibility, and environmental needs

  • Coordinating with individuals, families, guardians, support coordinators, and other involved professionals

  • Supporting compatibility and household considerations

  • Helping individuals understand their rights and responsibilities within the home

  • Supporting adjustment to a new living environment

  • Encouraging participation in household routines and community activities

  • Monitoring the placement to help identify concerns, changing needs, or additional supports

  • Supporting goals identified in the individual’s service plan

  • Promoting stability, independence, and long-term success in the chosen living arrangement
     

At Bloom Community Supports LLC, we understand that where a person lives can have a major impact on their comfort, independence, relationships, and overall well-being. Semi-Independent Living (SIL)

Semi-Independent Living at Bloom Community Supports LLC is designed to support individuals with intellectual and developmental disabilities who are able to live with a greater level of independence but may still benefit from structured assistance, guidance, and ongoing support with certain areas of daily life.
 

Services are tailored to each person’s strengths, preferences, goals, abilities, and individualized service plan. Our staff focus on helping individuals strengthen the skills they need to manage their daily routines, make informed choices, participate in their communities, and maintain a safe and stable living environment with as much independence as possible.
 

Semi-Independent Living may include support with:

  • Managing daily routines and personal responsibilities

  • Meal planning, grocery shopping, and meal preparation

  • Household organization and basic home-management tasks

  • Personal care and hygiene routines, when needed

  • Budgeting, money-management skills, and responsible spending

  • Scheduling appointments and managing important activities

  • Transportation planning and community access

  • Community participation and social involvement

  • Communication and interpersonal skills

  • Decision-making and problem-solving

  • Safety awareness and emergency preparedness

  • Developing and maintaining healthy routines

  • Building independent living and self-management skills

  • Understanding rights, responsibilities, and personal choices

  • Supporting goals identified in the individual’s service plan

  • Encouraging meaningful relationships, hobbies, recreation, and community involvement
     

Our staff provide guidance and assistance in a way that supports independence rather than creating unnecessary dependence. The goal is to help each individual take an active role in managing their own life, make choices with confidence, and develop the skills needed to maintain greater control over their daily routines and living environment.
 

At Bloom Community Supports LLC, Semi-Independent Living is built around the belief that individuals should be supported to do as much for themselves as they are able, while still having access to reliable assistance when needed.
 

We work with each person to strengthen confidence, stability, safety, and self-determination while promoting continued growth toward greater independence.

Our goal is to help each person live with greater freedom, confidence, responsibility, and connection to their community while receiving the level of support that is right for them.
